If performing embolization procedures using a pressure generating catheter with a one-way valve (you may hear this defined as a “TriNav Catheter”), hospitals and ASC’s need to be aware of the 3 Level ll HCPCS codes listed below. These codes are not to be listed on a CMS 1500 form, but only on the UB-04 form.
C1982 Catheter, pressure-generating, one-way valve, intermittently occlusive
C9797 Vascular embolization or occlusion procedure with use of a pressure-generatingcatheter (e.g., one-way valve, intermittently occluding), inclusive of all radiological supervision and
interpretation, intraprocedural roadmapping, and imaging guidance necessary to complete the intervention; for tumors, organ ischemia, or infarction
C8004 Simulation angiogram with use of a pressure-generating catheter (e.g., one-wayvalve, intermittently occluding), inclusive of all radiological supervision and interpretation, intraprocedural roadmapping, and imaging guidance necessary to complete the angiogram, for subsequent therapeutic radioembolization of tumors
C1982 describes the supply itself (i.e., the catheter).
C8004 describes angiography (i.e., the imaging) on a date separate than the embolization.
C9797 describes the embolization procedure itself and would be submitted instead of code 37242 or 37243.
Note: You should never submit codes C8004 and C9797 at the same encounter.
